Elders who have urgent needs but do not have the means to access necessary medical equipment; or/ and disabled persons who have urgent needs but do not have the means to install facilities for rehabilitation at home.
Applicants must be referred by Local Executive/ Social Welfare Department/ Voluntary Organizations
